29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  (a)  At 10113 South Ryan Creek Court, Franklin, Wisconsin, an employee was conducting carpentry work, on a residential building, and utilized a forklift with a basket to access elevated areas. The basket was not secured to the forklift to prevent it from displacement. This exposed an employee to a fall hazard greater than 10 feet from the lower level.

The employee was not using a harness and lanyard or other approved means of fall protection. This exposed an employee to a fall hazard greater than 10 feet from the lower level.

The operator of the forklift was not trained and or certified to operate the forklift (in accordance with 29 CFR 1910.178(l)). This exposed employees to struck-by, caught-in-between, and fall hazards.
