29 CFR 1926.302(b)(2):Safety clips or retainers shall be securely installed and maintained on pneumatic impact (percussion) tools to prevent attachments from being accidentally expelled. a) Work site was located on W 8th Street between John F. Kennedy Blvd. and Avenue C, Bayonne, NJ 07002 An employee was exposed to struck by hazards as they were working with a Hokuetsu Industries Portable Compressor and a Paving Breaker without using whip checks on the connections to prevent uncontrolled lashing of the air line connection. Violation was observed on or about April 14, 2021.

29 CFR 1926.601(b)(14): All vehicles in use were not checked at the beginning of each shift to assure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage which could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices. a) Work site was located on W 8th Street between John F. Kennedy Blvd. and Avenue C, Bayonne, NJ 07002 Employees were exposed to struck by hazards as the Komatsu WA-250 Loader they were working with in the work zone had inoperative reverse lights and one brake light out. Violation was observed on or about April 14, 2021.
