29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about April 19, 2021, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance activities on the plastic injection molding machine, saws, orbit former machines, grinders, and other machines and equipment at the facility were protected from unexpected energizing, startup, or release of stored energy.  Machines and equipment used throughout the facility are powered by electric, pneumatic, and hydraulic power.

29 CFR  1910.178(l)(1)(ii):Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  On or about April 19, 2021, the employer did not ensure employees operating powered industrial trucks (forklifts) had successfully completed powered industrial truck safe operation training.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   On or about April 19, 2021, the employer did not ensure all portions of a Tannewitz, Type GH, Vertical Band Saw blade, except the working portion of the blade between the bottom of the guide rolls and the table, were guarded to protect operators from contacting the rotating blade.

On or about April 19, 2021, the employer did not ensure the Farrel Plastic Injection Molding Press No. 3 was equipped with a disconnecting means capable of being locked in the open position.  The electrical disconnect lever was broken and/or missing from the machine.

29 CFR  1910.28(b)(11)(ii):(ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2)  On or about April 19, 2021, the employer did not ensure a flight of stairs located at the HPM Plastic Injection Molding Machine, consisting of 5 treads and 5 risers, was equipped with a stair rail system on each open side.
