29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 04/20/2021 employees at the Vine Street roofing project were exposed to falls while engaged in roofing activities without fall protection in the following locations: A) Employees were exposed to approximately 18' exterior falls while ripping roofing materials from a 8:12 pitched roof. The employees were not provided fall protection. B) Employees were exposed to falls of approximately 10' while ripping roofing materials from a 4:12 pitched roof. The employees were not provided fall protection.

29 CFR 1926.552(a)(1):29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. On or about 04/20/2021 employees at the Vine Street roofing project were using their RGC PRO 200 TRAC materials hoist not in accordance with the manufacturer specifications and limitations. The employees were using the hoist tracks as a ladder.
