On or about August 15, 2025 the employer did not ensure that employees operating a Metabo HPT NV 45AB2 Coil Nailer used appropriate eye protection when exposed to hazards from flying particles.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  31 Elvree Street Manchester, CT. On or about August 15, 2025 the following conditions were observed during an inspection: a) A plank used for a roof bracket scaffold was split for approximately three feet of its length and overhung approximately four feet beyond the end bracket and roof edge. b) Planking used in a ladder jack scaffold was comprised of different materials and stacked in a manner that created an uneven and unstable walking surface.
