29 CFR 1910.212(a)(2):Guards on machine were not affixed to the machine or secured elsewhere when attachment to the machine was not possible On or about 4/19/2021, at the Sock Tucker, located at 1005 Valley Dr. in the Perry, GA facility, employees were exposed to a an amputation hazard from moving parts, when the employer failed to ensure the metal gate/access door used as guarding around the equipment was securely affixed to the machine, where possible, or fastened by any method (e.g., bolts) that prevented the guard from being inadvertently dislodged or removed.
