29 CFR  1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.  a) Commissary Warehouse:  On or about April 20, 2021, employees used stair ladders with damaged steps and structural cross members to retrieve items stored in the aisle, exposing employees to fall hazards.

29 CFR  1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater:  a)  Dry storage warehouse, Door C:  On and or about April 20, 2021, pallet containing materials were place in front of the electrical panel  restricting working space and blocking access to the panel, exposing employees to fire and electrical hazards.  b)  Dry storage warehouse, Door D:  On and or about April 20, 2021, pallet containing materials were place in front of the electrical panel  restricting working space and blocking access to the panel, exposing employees to fire and electrical hazards.  c)  Dry storage warehouse, Door E:  On and or about April 20, 2021, pallet containing materials were place in front of the electrical panel  restricting working space and blocking access to the panel, exposing employees to fire and electrical hazards.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(i)(B) ), which was contained in OSHA inspection number 1524271, citation number 1,  item number 3 and was affirmed as a final order on May 25, 2021, with respect to a workplace located at 689 North Terminal Road San Juan, Puerto Rico.
