29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Site: Extrusion department, cutting machine, (puller Andon), Model: Custom- AccuFly, Serial Number 1626170: On or about April 22, 2021, rubber and yarn cutting machine was not effectively guarded. A panning operator suffered amputations of three of his fingers while reaching in to the blade to unjam rubber and yarn. When the worker removed the rubber, a 24 inch guillotine type blade activated and cut his fingers.
