29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or before April 28, 2021, the employer failed to ensure employees utilized the written energy control procedure for the Rebecca Line Scatter Unit for the adjustment of the spread width and auger blades width. Supervisors did not enforce the energy control procedure as operators routinely adjusted the scatter unit width and auger width during normal production operations. This practice exposed employees to caught-in hazards of the scatter unit.

On or before April 28, 2021, the employer did not ensure that each authorized employee affixed a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when employees began work, and removed those devices when the employee stopped working on the machine or equipment being serviced or maintained. Employees worked with maintenance personnel inside the laminator machine without participating in the group lockout of the machine, thereby exposed to caught-in hazards.

On or before April 28, 2021, the employer did not provide training on the energy control program and procedures to be used by employees who performed servicing and maintenance on equipment such as, but not limited to, the Rebecca Line laminator. b. On or before April 28, 2021, the employer did not provide training on the energy control program and procedures to be used by employees who performed servicing and maintenance on equipment such as, but not limited to, the Rebecca Line Scatter Unit. Ridge Corporation was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1172671, citation number 1, item number 1 and was affirmed as a final order on February 7, 2018 with respect to a workplace located at 5777 Raiders Rd, FRAZEYSBURG, OH 43822.
