On or before January 10, 2022, the employer did not ensure that each powered industrial truck operator was competent to operate the powered industrial truck safely. The employer did not evaluate an employee's competency when they operated the powered industrial vehicle such as, but not limited to the JLG, Skytrack, Rough Terrain Forklift at the worksite. As a result employees were exposed to struck-by hazards.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel. a. On or about January 10, 2022, the employer did not ensure that the Toyota Electric Forklift Truck, Model 8BRU18, Serial No. 8BRU18-38202 was taken out of service when it was identified that the vehicle leaked hydraulic fluid and was not in a safe operating condition. Employees were exposed to struck-by hazards as a result.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a. On or before January 10, 2022, the employer did not ensure that the JLG, Skytrack, Rough Terrain Forklift that was in need of repair for the back-up alarm and back-up light was taken out of service. Employees were exposed to struck-by hazards as a result. Ridge Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1), which was contained in OSHA inspection number 1146774, citation number 1, item number 3 and was affirmed as a final order on February 07, 2018, with respect to a workplace located at 1201 Etna Parkway, Pataskala, Ohio 43062.
