29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about April 1, 2021, at the worksite located at 1755 Olympic Drive Athens, Georgia, employees were exposed to struck-by hazards, when the employer failed to obtain prior written approval from the forklift manufacture before utilizing an employer installed side shift and long probe. The probe was used to move the completed rolls of industrial netting material, from the manufacturing area to the warehouse area.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory-installed attachments, however, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about April 1, 2021 at the worksite located at 1755 Olympic Drive in Athens Georgia employees were exposed to struck-by hazards when the employer failed to ensure that the forklift utilizing the employer purchased and installed attachment was appropriately marked according to this standard.
