29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  New residential construction located on Lot #46 White Spruce Way, Newnan, GA 30265, Roof: On or about 04/28/2021, the employer exposed employees to struck-by hazards while operating pneumatic nails guns, in that protective eyewear was not provided to employees prior to operating these tools at 150 PSI engaged in roofing activities.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    New residential construction located on Lot #46 White Spruce Way, Newnan, GA 30265, Roof: On or about 04/28/2021, the employer exposed employees to fall hazards of 10-feet, 4-inches to the lower ground level, in that employees wearing full body harnesses were not attached to life-lines and/or lanyards while engaged in roofing activities on a 6/12 pitched roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    New residential construction located on Lot #46 White Spruce Way, Newnan, GA 30265, Front side of structure: On or about 04/28/2021, the employer exposed employees to fall hazards of up to 10-feet, 4-inches to the lower ground level, in that employees a ladder extended at least 3-feet and/or secured to the structure was not provided to employees accessing the roof engaged in roofing activities on a 6/12 pitched roof.
