29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the such employee. The employer did not provide the respirators which are applicable and suitable for the purpose intended. The employer did not establish a respiratory protection program including the requirements outlined in paragraph (c) of this section, to cover each employee required by this section to use a respirator: a) New Jersey Addiction Treatment Center LLC d/b/a Sunrise House located at 37 Sunset Inn Rd, Lafayette, NJ On or about 04/27/2021, the employer did not provide N95 respirators to protect employees exposed to SARS-CoV-2. Employees working in Admissions along with Registered Nurses and Licensed Practical Nurses were exposed to SARS-CoV-2 while admitting patients and providing direct patient care to suspected and confirmed COVID-19 patients. New Jersey Addiction Treatment Center LLC d/b/a Sunrise House was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(a)(2), which was contained in OSHA inspection number 1473911, citation number 1, item number 1 and was affirmed as a final order on 04/05/2021, with respect to a workplace located at 37 Sunset Inn Rd, Lafayette, NJ.
