29 CFR 1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked: a) On or about May 3, 2021 in the Carton Storage area, palletized cartons double stacked in the main aisle way did not provide sufficient clearance for forklifts to operate within the aisle way. b) On or about May 3, 2021 in the Receiving Area, multiple stacks of roll stock were staged in close proximity to the overhead plumbed gas lines and would not provide sufficient clearance for a clamp truck to stack and/or unstack roll stock from the main aisle way.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: On or about May 3, 2021 palletized flat cartoons that were double stacked and staged in the aisle way adjacent to the emergency exit door in the Carton Storage Area were not stacked in a stable manner and exposed employees to a potential struck by hazard.

29 CFR 1910.176(e): Clearance limits. Clearance signs to warn of clearance limits shall be provided: a) On or about May 3, 2021 clearance limit signs were not in place for the side of the plumbed gas lines feeding the ceiling mounted heaters along the main aisle way in the Receiving Area. Stacked paper roll stock was stacked and unstacked in close proximity to the gas lines. b) On or about May 3, 2021 a clearance limit sign was not in place to identify clearance limits to the elevated transformer enclosure installed on the building in the Roll Wip Stock area. Paper roll stock was stacked and unstacked in close proximity to the elevated enclosure.

29 CFR 1910.303(g)(1)(i)(A): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed: On or about May 3, 2021 in the Paper Roll Stock area, the working space three feet in front of the electrical panel, within the building column, was not kept clear for immediate access. Large paper rolls were staged immediately in front of the panel, blocking access.
