29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:  On or about May 6, 2021, and times prior thereto, employees were exposed to fall hazard while working on scaffolds that did not have planks installed covering the width of scaffold.

29 CFR  1926.451(b)(7):On scaffolds where platforms are overlapped to create a long platform, the overlap was not at least 12 inches (30 cm) and the platforms were not nailed together or otherwise restrained to prevent movement:  On or about May 6, 2021, and times prior thereto, employees were exposed to a fall hazard while working on scaffolds that did not have adequate overlapping planks at joining sections.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not use:  On or about May 6, 2021, and times prior thereto, employees were exposed to a fall hazard while using scaffold planks as a ramp and supported on the top of cross braces for access and egress.

29 CFR  1926.452(c)(2): All cross braces were not of such length so as to automatically square and align vertical members such that the erected scaffold was always plumb, level, and square:  On or about May 6, 2021 and times prior thereto, employees were exposed to fall hazard while working on scaffold that did not have cross bracing installed securely on all sections in accordance with manufacturer's instructions and scaffold type.
