29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. On or about May 6, 2021, during the installation of roofing material at a job site located at 1904 Lincoln Way East, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 8.5 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(4):Ladders shall be used only for the purpose for which they were designed. On or about May 6, 2021, during the installation of roofing material at a job site located at 1904 Lincoln Way East, Massillon, Ohio 44646; during the installation of roofing material on a residential roof, the employer did not ensure that employees utilized self-supporting ladders as designed by the manufacture. Employees utilized a collapsed self-supported ladder as means of access and egress to an elevated working surface by leaning a collapsed self-supporting ladder against the roofs edge which was approximately 8.5 feet above the ground.
