29 CFR  1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  a) Avnet Machine #3; On or about April 29, 2021 the employer exposed employees to  machine hazards associated moving machine parts and in-going nip points in that employees were required to troubleshoot - test or position - the die in Machine #3 while the machine was energized. The employer failed to follow the required sequence of actions as set by the standard when equipment is energized for testing or positioning. Employees were not removed from the machine or equipment area prior to the machine being energized.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Avnet Machine #6 ; On or about May 4, 2021 and at times prior to employees were exposed to machine hazards associated moving machine parts and in-going nip points while they were replacing spacers within the guarded portion of the Avnet #6 die cast machine. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation from the energy source [(d)(3)], and securing LOTO device to the energy isolating device (disconnect) [(d)(4)(i)] were not implemented to protect employees from machine servicing hazards.
