29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Jobsite, 602 Thomas Road, Monroeville, PA 15146: On or about May 10, 2021 employees did not ensure that devices needed to control the energy to a Smithco brand air cooled ammonia condenser were physically located and operated in a manner as to isolate the equipment from the energy sources. An employee received serious injuries from exposure to ammonia that was released after a plug was removed from a Smithco brand air cooled ammonia condenser located outside the building.

Heat Exchangers, LLC) were engaged in maintenance activities on a Smithco brand air cooled ammonia condenser at this facility, the onsite employer (Montauk Energy Holdings, LLC) and the outside employer did not inform each other of their respective lockout or tagout procedures.
