29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Armstrong World Industries, Inc located at 451 E Coshocton St Johnstown, OH 43031: On and preceding May 11, 2021, The employer did not ensure that machine guarding was provided to protect the operator of a Niagra Perforator machine at the material infeed and table where material is punched. A perforator used for punching holes in spools of metal was not guarded in the area where the material is fed into the machine or the area where the machine punches that metal. The following condition exposed employees to caught-in hazards. b) Armstrong World Industries, Inc located at 25 Greenscape Ct, Johnstown, OH 43031: On and preceding May 11, 2021, The employer did not ensure that machine guarding was provided to protect employees from nip points on the tube line conveyor. A conveyor used for movement of work pieces which are glued as they move on the conveyor had unprotected nip points where the rollers and belt met near the location where the operator was positioned. The following condition exposed employees to caught-in hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Armstrong World Industries, Inc located at 451 E Coshocton St Johnstown, OH 43031: On and preceding May 11, 2021, the employer did not ensure listed or labeled electrical equipment was used or installed in accordance with instructions included in the listing or labeling. A miter saw that was located in the back of the facility near the loading docks and periodically utilized for building and modifying shipping containers was powered using a flexible cord which was plugged into a power strip in series (daisy chained). This condition exposed the employees to a fire hazard. b) Armstrong World Industries, Inc located at 25 Greenscape Ct, Johnstown, OH 43031: On and preceding May 11, 2021, the employer did not ensure listed or labeled electrical equipment was used or installed in accordance with instructions included in the listing or labeling. Miter saws and fans which were periodically utilized were powered using a flexible cords which were plugged into a power strips or additional flexible cords in series (daisy chained). This condition exposed the employees to a fire hazard.
