29 CFR 1910.37(a)(3):Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a) Armstrong World Industries, Inc located at 25 Greenscape Ct. Johnstown, OH 43031: On and preceding October 3, 2022, the employer did not ensure that exits routes were unobstructed. Employees use various machinery and operate a forklift throughout the facility. The path of travel and exit door leading out of the northeast exit was partially blocked by large boxes and a wooden crate which left an 18 inch opening at the point of the exit door. The following condition exposed employees to fire hazards.

29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit." a) Armstrong World Industries, Inc located at 25 Greenscape Ct. Johnstown, OH 43031: On and preceding October 3, 2022, the employer did not ensure that each exit was clearly visible and marked by a sign reading "exit". Employees use various machinery and operate a forklift throughout the production area where the east exit access door leading through the office area to the primary exit discharge was the only unobstructed exit on the east end of the building. The east exit access door was not marked with an exit sign facing into the production area. The following condition exposed employees to fire hazards.

Johnstown, OH 43031: On and preceding October 3, 2022, the employer did not ensure that powered industrial truck operators completed training and evaluation prior to operating forklifts to load/offload trucks and move materials throughout the warehouse. At the time of the inspection, an active operator of the forklift in the facility had not completed training and/or evaluation on the use of the forklift thereby exposing employees to struck-by hazards.

Johnstown, OH 43031: On and preceding October 3, 2022, the employer did not ensure that an in service out within the production area was used according to the listing and labeling. A 110v outlet box that was in use powering a water cooler was hanging by the flexible conduit and not mounted to the wall thereby exposing employees to electrical shock hazards.
