29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:  On or about May 5, 2021 employees were exposed to struck-by hazards while removing the top portion of a clam-shell clamp from a Vestas V150 blade fixture.  The employees free rigged the sling onto the tines of a rough terrain forklift.  This operation affects the capacity and safe operation of the rough terrain forklift and the employer did not ensure that written approval was obtained from the manufacturer prior to the lift.
