29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 11, 2021, employees working in the wastewater area were exposed to amputation hazards because an ingoing nip created by the belt and roller at the end of the hair conveyor was not guarded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: On or about May 11, 2021, employees working in the wastewater area were exposed to electric shock and fire hazards because: a. A compression fitting used to connect electrical conduit sections was disconnected and the conduit separated below an HDI disconnect switch that controlled the auger motor of the red waste hopper. b. A compression fitting used to connect electrical conduit sections was disconnected and the conduit separated near the 7.5 horsepower motor that powered the red waste hopper auger.
