29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  a) Side of the Building: A JLG All Terrain Forklift was left unattended with the hoisting boom fully extended up to the fourth floor with a load of plywood, on or about 5/7/21.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Interior of Building - back: Employees were exposed to approximately 12 feet fall hazard while utilizing an extension ladder that only extend approximately 18 inches over the working surface, on or about 5/721.
