29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Interior of Building - Fourth Floor - Front:  Employee without any means of head protection were exposed to head injury while working below carpenters moving  framing lumber, on or about 8/13/21.

29 CFR  1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) Exterior - Front: Employees were exposed to electric hazards while unloading a boom truck that was operating approximately 3 feet from the 4KV Primary , on or about 8/13/21.

29 CFR  1926.416(a)(3): Employer did not post or maintain proper warning signs at locations where the performance of the work could bring a person, tool, or machine into physical or electrical contact with an energized electrical power circuit:   a) Exterior - Front: No signage was posted to advise/warn employees engaged in framing  activities while unloading a boom truck regarding the proximity of 3 feet or less from the energized 4KV Primary  line, on or about 8/13/21.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Exterior of Building - Fifth Floor: Employees without any means of fall protection were exposed to approximately 45 feet fall hazard while engaged in framing activities, on or about 8/13/21.
