Alliance, Springfield, MO: Employees were exposed to electrical hazards.       a: Two lockout tagout devices did not identify the employee applying the lockout tagout device when locking out the 460 Volt Main electrical panel along the South Wall in Final Assembly.  b: One lockout tagout device did not identify the employee applying the lockout tagout device to the 230 Volt Three phase electrical panel mounted to blue sheet metal in Final Assembly.  c: One lockout tagout device did not identify the employee applying the lockout tagout device to the 208 volt three phase electrical panel mounted to blue sheet metal in Final Assembly.

Alliance, Springfield, MO: An employee was exposed to electrical hazards.  An employee was required to test industrial washers which were connected 460 volts of electricity and did not receive authorized employee training on Lockout tagout training including training on when lockout tagout training was required, recognition of the hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  At a worksite located at 1435 N. Alliance, Springfield, MO:  Employees were exposed to electrical hazards. Employees in the Final Assembly Area, were using the 460 volt electrical box attached to blue sheet metal to test parts washing machines. The Orange Frontal Rotary Handle Disconnect Switch for the 460 volt electrical box was not connected to the interior T handle and would not disconnect the power. On or about May 12, 2021, an employee was injured by electrical shock when the electrical wiring cable he was going to use to test an industrial washing machine was live.

Alliance, Springfield, MO:  Employees were exposed to electrical hazards.  a:  There was an open knockout on the top of the  230 Volt three phase electrical breaker box attached to the blue sheet metal in Final Assembly/Testing.  b:  There was an open knockout on the left side facing the 460 volt main panel box against the South Wall of Final Assembly/Testing.
