29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about May 20, 2021, and at time prior thereto, two employees were using a pneumatic nail gun to secure shingles to a roof without wearing eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13):Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems. On or about March 20, 2021, and at times prior thereto, one employee and their supervisor were installing shingles on the roof of a residential home under new construction up to approximately 35 feet high without appropriate fall protection, exposing the employees to a fall hazard.
