29 CFR 1926.102(a)(2): Eye and face protection equipment did not meet the requirements of ANSI Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:  (a) On or about May 26, 2021, at least two employees roofing a house using pneumatic nail guns were wearing sunglasses that were not ANSI Z87.1 compliant and were exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about May 26, 2021 employees working on a steep roof were not protected from a fall hazard of up to approximately seventeen (17) feet.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about May 26, 2021, employees using a 16 foot Werner aluminum extension ladder on the front of the house placed with its side rails approximately six inches above the roofline were exposed to a fall hazard of approximately eight feet to the ground below while accessing the roof.  (b) On or about May 26, 2021, employees using the top section of an aluminum extension ladder on the side of the house placed with its side rails approximately two feet above the roofline were exposed to a fall hazard of approximately eight feet to the ground below while accessing the roof.  (c) On or about May 26, 2021, employees using a Werner 18 foot aluminum extension ladder at the back of the house placed with its side rails approximately one foot and six inches above the roofline were exposed to a fall hazard of approximately eight feet to the ground below while accessing the roof.
