29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Future Site of Samford Glen Apartments (Southeast Complex: East Siding Area): On or about May 19, 2021, the employer failed to ensure exposed employee(s) were using a body belt with lanyard attached to the approved anchor point while using a telescopic boom aerial lift to perform caulking/painting construction activities; exposing employee(s) to catapult hazards and fall hazards of approximately thirty (30) feet to the ground below.
