The employer allowed employees to ride on powered industrial trucks that were not equipped with a designated seat or an approved riding platform. The forklift was not designed to carry passengers, and no modifications had been made to safely accommodate riders, exposing the employee to the risk of falling or being crushed during movement or tipping.
