Moore Ave. Philadelphia, PA - 19121: The employer did not ensure that a floor hole in the walking/working surface of the first floor of the structure was properly guarded or covered, thus exposing employees to a fall hazard of approximately 7 feet to foundation level of the site.  Condition observed on or about June 1, 2021.

Moore Ave. Philadelphia, PA - 19121: The employer did not ensure that the 6 foot Portable A-Framed Werner ladder was used, per manufacturer's design. Employees used the ladder as a means for entry and exit for the building in a closed position and leaned against the wall structure prior to its use, thus exposing employees to a fall hazard of approximately 5 feet to ground level. Employees were conducting framing activities at a height of approximately 14 feet without the use of an adequate fall protection system.  Condition was observed on or about June 1, 2021  AE Carpenters, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1406812, Citation Number 1, Item Number 2 and was affirmed as a final order on December 24, 2019, with respect to a workplace located at 1206 N Oriana St, Philadelphia, PA- 19122.  AE Carpenters, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1291667, Citation Number 2, Item Number 2 and was affirmed as a final order on June 20, 2018, with respect to a workplace located at 433 North 35th Street. Philadelphia, PA - 19104.
