29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about May 22, 2021, the employer had not developed machine specific lockout procedures for the Riverside saw dust conveyor and/or the saw dust dryer to be utilized by authorized employees for servicing and/or maintenance tasks.

29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage: On or about May 22, 2021, the authorized employee performing a belt change on the Riverside saw dust conveyor did not utilize the normal shut down procedures for the equipment. The employee turned off the electrical disconnect which permitted the equipment to restart immediately when the disconnect was turned back on.

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and de-energization of the machine or equipment have been accomplished: On or about May 22, 2021, prior to initiating the belt change on the Riverside saw dust conveyor, the authorized employee did not verify that the conveyor system had been effectively disconnected from the electrical power source and could not be restarted during the course of the servicing task.
