29 CFR 1910.22(a)(1):The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. Pretium Packaging, LLC - Leipsic, Ohio: On or about June 2, 2021, the employer did not ensure the work floor at the grinders near machines such as, but not limited to, blow molding machines H, I, and J, was kept clean. Employees that access the area and/or empty the regrind hoppers were exposed to same level slip/fall hazards due to plastic regrind material on the floor. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
