1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about 6/2/2021 at NW 103rd Terrace and 106th Terrace, Pembroke Pines, FL 33023, the employer did not implement and maintain an effective accident prevention program at the worksite, in that the employees of a subcontractor were permitted to perform roofing repair work (tear off and shingle installation) without the use of fall protection, thus exposing employees to a 12 to 28 feet fall hazard.

1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) On or about 6/2/2021 at NW 103rd Terrace and 106th Terrace, Pembroke Pines, FL 33023, employees of a subcontractor were observed working on two story multi-family residential complex buildings without a fall protection system, resulting in the employees' exposure to a 12 to 28 feet fall hazard while engaged in roof repair work - tear off and shingle installation. b) On or about 6/8/2021 at 421 NW 106th Terrace, Pembroke Pines, FL 33023, employees of a subcontractor were observed working on a two story multi-family residential complex building without a fall protection system, resulting in the employees' exposure to a 12 to 28 feet fall hazard while engaged in roof repair work - tear off and shingle installation. c) On or about 6/8/2021 at 421 NW 106th Terrace, Pembroke Pines, FL 33023, an employee was observed on a roof examining and overseeing the work of a roofing subcontractor without utilizing a fall protection system, thus resulting in an exposure to a 12 to 28 feet fall hazard.
