29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about June 1, 2021 the employer failed to ensure that employees tasked with operation, service, and maintenance of the 202-11 envelope production line had received training as authorized employees for the control of hazardous energies. Employees operated the energy isolation points in a manner to isolate the hazardous energies present during a belt change.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer failed to enforce the energy control procedure as operators routinely performed service and maintenance during both normal production and non production operations. These service and maintenance practices exposed employees to moving machine parts and in-going nip point hazards created by the moving machine components. a) On or about June 1, 2021 the employer failed to ensure that authorized employees affixed lock out devices to the isolation points of the envelope 202-10 line prior to performing service and maintenance on the equipment. An employee utilized a special tool in order to gain access to a locked panel on the backside of the machine prior to performing the steps outlined in the procedures. The employee was not afforded additional protection from the hazardous area of the moving machine parts including belts and rotating shafts while the equipment was in operation. b) On or about June 1, 2021 the employer failed to ensure that authorized employees affixed lock out devices to the isolation points of the envelope 202-11 line prior to performing service and maintenance on the equipment. An operator performed service and maintenance in the form of a belt change on the production line. As part of this procedure the operator placed the main disconnect in the off position without affixing a lockout device or completing the remaining control elements as described in the procedures.
