29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about 4/10/19, the employer failed to ensure that periodic inspections of the procedures for the control of hazardous energies were performed. Periodic inspections were not performed for any equipment within the facility in which employees would be required to lock out and tag out. Hazardous energy sources include but are not limited to electrical, mechanical, hydraulic, and pneumatic. Equipment within the facility includes but is not limited to, envelope presses, slitters, and balers. Envelope 1 was previously cited for a violation an equivalent occupational safety and health standard 29 CFR 1910.147(c)(6)(i)(C), which was contained in OSHA inspection number 1074333, citation number 2, item number 1 and was affirmed as a final order on 12/22/16, with respect to a workplace located at 41969 State Route 344, Columbiana, OH 44408.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): The employer failed to ensure that the electrical, pneumatic, and mechanical energy disconnects for the envelope lines were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each specific energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during servicing/ maintenance and set up operations. a) On or about 6/7/19, Envelope Line 202-8, the electrical, pneumatic, and vacuum sources were not locked out. b) On or about 6/10/19, Envelope Line 202-1, the electrical, pneumatic, and vacuum sources were not locked out, c) On or about 6/10/19, Envelope Line 102-3, the pneumatics, and vacuum sources were not locked out, d) On or about 6/10/19, Envelope Line 102-10, the electrical, pneumatic, and vacuum sources were not locked out, e) On or about 6/10/19, Envelope Line 102-11, the pneumatic and vacuum sources were not locked out, and f) On or about 6/10/19, Envelope Line 202-7, the electrical, pneumatic, and vacuum sources were not locked out. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 6/7/19, the employer failed to ensure that machine guarding methods were in place to prevent accidental contact with moving parts. The Hamblet Mach. Co. paper sheeter had a missing guard on the side with the ladder that leads to the catwalk above by the red control panel. Envelope 1 was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1074333, citation number 1, item number 1 and was affirmed as a final order on 12/22/16, with respect to a workplace located at 41969 State Route 344, Columbiana, OH 44408.
