Building Systems, Inc., 150 West 10th Street, Erie, PA 16501, fourth floor, on or about June 9, 2021 and times prior thereto: A 4-1/2-inch floor hole was equipped with a cover that was not secured to the floor. B.) Building Systems, Inc., 150 West 10th Street, Erie, PA 16501, fourth floor, on or about June 9, 2021 and times prior thereto: A 1-1/2-inch floor hole was equipped with a displaced cover that was not turned over the floor hole and was not secured to the floor as it was missing a second screw that was meant to anchored the cover to the floor. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
