29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system: (a) Location - 67 Faulkner Street, North Billerica, MA: On or about May 19, 2021, employees were exposed to falling off of the unprotected edge of a lower roof while using a fixed industrial ladder to gain access to an upper roof during a response to a fire. (b) Location - 67 Faulkner Street, North Billerica, MA: On or about May 19, 2021, an employee was exposed to falling off the unprotected edge of a roof while suppressing a fire. (c) Location - 67 Faulkner Street, North Billerica, MA: On or about May 19, 2021, an employee was exposed to falling off of the unprotected edge of an upper roof while using a fixed industrial ladder to egress from the upper roof following a response to a fire.
