On or about June 11, 2021, on the northside of roof of a Rooms to Go Distribution Center located in Brookshire, Texas, employees were exposed to fall hazards while engaged in installing a gravel guard at the leading edge of the roof with unprotected side and edges without a personal fall arrest system.
