29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a. On or about June 15, 2021, at the above addressed jobsite, the ladder used to access and egress the excavation was not long enough to extend three (3) feet above the upper landing surface and was not secured at its top and a grasping device was not provided, thereby exposing an employee to fall hazards. b. On or about June 28, 2021, at the above addressed jobsite, the ladder used to access and egress the excavation was not long enough to extend three (3) feet above the upper landing surface and was not secured at its top and a grasping device was not provided, thereby exposing an employee to fall hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: a. On or about June 28, 2021, at the above addressed jobsite, employees working in and around an excavation with new concrete foundation footings were exposed to protruding ends of rebar that were not guarded, thereby exposing employees to impalement hazards. Og Plumbing LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.701(b), which was contained in OSHA inspection number 1467549, citation number 1, item number 3, and was affirmed as a final order on May 4, 2020, with respect to a workplace located at 2346 W. Grand Ave. in Chicago, Illinois.
