29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) The employer failed to provide documentation demonstrating the abatement method and completion for employees conducting roofing activities were not protected from falling to lower lover issued on 9-3-2021 as Citation 1 Item 1  (b) The employer failed to provide documentation demonstrating the abatement method and completion for employees trained to recognize and minimize fall hazards issued on 9-3-2021 as Citation 1 Item 2.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  On or about June 17, 2021, employees conducting roofing activities on a roof with a 16:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 13 feet.  The Marcos Hernandez Martinez dba Marcos Hernandez Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1352562, citation number 1, item number 2 and was affirmed as a final order on February 20, 2019, with respect to a workplace located at 1500 Serena Lane, Burlington, WI 53105.   The Marcos Hernandez Martinez dba Marcos Hernandez Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1463681, citation number 2, item number 1 and was affirmed as a final order on July 29, 2020, with respect to a workplace located at 216 Oak Hill Circle, Burlington, WI 53105.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about , June 17, 2021, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall hazard of approximately 13 feet.
