29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation: The employer is failing to protect employees from caught in injuries while clearing jams on machinery that is not locked out. This was most recently documented on June 6, 2021 at 555 Stuhr Rd, Grand Island, Nebraska where an employee received serious injuries while attempting to remove a jammed hide from a flesher machine without first locking it out.
