29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a. Scaffolding on the south side of the library: Employees performing soffit work were standing on a scaffolding that was missing a mid-rail and a top-rail on the west end of the scaffolding, exposing employees to fall hazards  b. Scaffolding on the west side of the library: The scaffolding employees were standing on while performing fascia repair work was missing a mid-rail and a top-rail on the south end of the scaffolding, exposing employees to fall hazards.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.  Southwest corner of roof: An employee applying  sealant to the roof was not protected from falling to the lower level. The employee was exposed to fall hazards.

29 CFR  1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  In front of the library:  A ladder used to access the roof did not extend above the roof and the top of the step ladder was used to access the roof. Employee was exposed to fall hazards.
