Employees were exposed to flying particulate as well as arm, hand, and body chemical burn hazards presented by mustard flour in the milling and bagging processes.

29 CFR 1910.219(c)(2)(i):  All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires:  At a facility located at 478 Industrial Park Road, Berlin, WI 54923; the employer did not protect or enclose horizontal shafting exposing employees to rotating parts less than seven (7) feet above the floor:  a)  Horizontal shafts powered by belts and pulleys on Mill #3 were not protected or enclosed.  b)  Horizontal shafts powered by belts and pulleys on Mill #5 were not protected or enclosed.  c)  Horizontal shafts powered by belts and pulleys on Mill #6 were not protected or enclosed.  The Wisconsin Spice Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.219(c)(2)(i), which was contained in OSHA inspection number 1207088, citation number 1, item number 2a and was affirmed as a final order on April 19, 2017, with respect to a workplace located at 478 Industrial Park Road, Berlin, WI 54923.

29 CFR 1910.219(d)(1):  Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.   At a facility located at 478 Industrial Park Road, Berlin, WI 54923; the employer did not guard pulleys less than seven (7) feet above the floor:  a)  Multiple pulleys on Mill #3 were not guarded to prevent contact with the rotating parts and spokes of the pulleys.  b)  Multiple pulleys on Mill #5 were not guarded to prevent contact with the rotating parts and spokes of the pulleys.  c)  Multiple pulleys on Mill #6 were not guarded to prevent contact with the rotating parts and spokes of the pulleys.  The Wisconsin Spice Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.219(d)(1), which was contained in OSHA inspection number 1207088, citation number 1, item number 2b and was affirmed as a final order on April 19, 2017, with respect to a workplace located at 478 Industrial Park Road, Berlin, WI 54923.

At a facility located at 478 Industrial Park Road, Berlin, WI 54923; the employer did not fully enclose belts or guard the in-running nip points of horizontal belts where they contacted pulleys less than seven (7) feet above the floor:  a)  Multiple belts on Mill #3 were not fully enclose or guarded belts  to prevent contact with the moving belts or the in-running nip-points at the pulleys.  b)  Multiple belts on Mill #5 were not fully enclose or guarded belts  to prevent contact with the moving belts or the in-running nip-points at the pulleys.  c)  Multiple belts on Mill #6 were not fully enclose or guarded belts  to prevent contact with the moving belts or the in-running nip-points at the pulleys.  The Wisconsin Spice Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.219(e)(1)(i), which was contained in OSHA inspection number 1207088, citation number 1, item number 2c and was affirmed as a final order on April 19, 2017, with respect to a workplace located at 478 Industrial Park Road, Berlin, WI 54923.

The pendant outlet was used to power a scale and workshop vacuum. The metal outlet box was not installed in accordance with the UL identifier QCIT, which states that boxes shall be mounted in or on a surface.
