29 CFR 1910.28(b)(3)(ii): 29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. Employees were exposed to tripping and fall hazards. The employees operating the grain elevator were exposed to trip and fall hazards. The 3rd floor of the head house had 7-floor spouts that were missing guards exposing employees in the area to trips and falls.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy. The employer failed to protect employees from hazards associated with the unexpected release of energy. This was most recently documented on June 11, 2021. The employer had not developed and implemented machine-specific procedures that outlined steps for de-energizing all the sources of energy for the extruder machine. In addition, alternative methods of protection were not provided to employees in the area to prevent exposure to steam hazards during servicing and maintenance operations.
