29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) DECA Commissary (1180 Amphibious Drive, VA 23459): On or about 06/25/2021, employees were assigned to prepare a bleach spray for disinfection of equipment for customer use.  Bleach preparation required employees to dilute concentrated bleach and pour the dilution into spray bottles.  The bleach preparation process was performed in the chemical sink by the warehouse.  The employer was unable to provide a hazard assessment identifying required PPE and/or controls for the bleach preparation process.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Bleach preparation required employees to dilute concentrated bleach and pour the dilution into spray bottles.  The bleach preparation process was performed in the chemical sink by the warehouse.  Employees were not required to and generally did not wear eye protection while performing the bleach dilution and pouring tasks.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a)	DECA Commissary (1180 Amphibious Drive, VA 23459): On or about 06/25/2021, employees were assigned to prepare a bleach spray for disinfection of equipment for customer use.  Bleach preparation required employees to dilute concentrated bleach and pour the dilution into spray bottles.  The bleach preparation process was performed in the chemical sink by the warehouse.  The nearest eye wash station to the bleach preparation area was approximately 40 feet away, through a door, and around a significant amount of storage material.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1)(i):The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet;  a) DECA Commissary (1180 Amphibious Drive, VA 23459): On or about 06/25/2021 and times prior thereto, the employer did not ensure that all products, containing hazardous chemicals, and used routinely by employees for COVID-19 sanitation procedures, were listed in the employer's hazardous chemical inventory list. Specifically, the Maxim Germicidal Cleaner did not appear on the chemical inventory list.  Corrected during the inspection.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s);  a) DECA Commissary (1180 Amphibious Drive, VA 23459): On or about 06/25/2021 and times prior thereto, the employer did not ensure that all Safety Data Sheets, associated with the cleaning chemicals used at their facility, were available and up-to-date. The Maxim Germicidal Cleaners used to prepare the disinfecting wipes, used routinely onsite, did not have an associated Safety Data Sheet in the chemical inventory binder.  Corrected during the inspection.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical;  a) DECA Commissary (1180 Amphibious Drive, VA 23459): On or about 06/25/2021 and times prior thereto, employees were operating secondary containers of hazardous cleaning chemicals, specifically four spray bottles filled with bleach, which were incompletely labelled to provide all required information regarding the physical and health hazards of the hazardous chemical.  Corrected during the inspection.
