(a)	Eldorado Wall Company, Inc. dba Eldorado Climbing at 1699 Cherry St., Unit B, Louisville, CO 80027:  On and before June 24, 2021, the employer did not ensure all forklift operators were trained in the operation of powered industrial trucks and that the operator's training consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace. This condition exposed employees to struck-by hazards when employees operated forklifts without having been adequately trained and evaluated.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a)	Eldorado Wall Company, Inc. dba Eldorado Climbing at 1699 Cherry St., Unit B, Louisville, CO 80027:  On and before June 24, 2021, the employer did not ensure that the dual wheel, bench grinder located in the steel shop had the correct distance between the wheel periphery and the edge of the work rests so that it did not exceed 1/8 inch. The work rest distance on the left side measured greater than 1 inch and the right side work rest was broken off. This condition exposed the employees to abrasion hazards to the hands.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:    (a)	Eldorado Wall Company, Inc. dba Eldorado Climbing at 1699 Cherry St., Unit B, Louisville, CO 80027:  On and before June 24, 2021, the employer did not ensure that the DeWALT (Model DWE402) 4.5 inch angle grinder in the steel shop was properly fitted with a guard over the portion of the blade closest to the user's hands to protect operators and other employees from hazards created by the rotating blade. The employees used this power tool to, but not limited to, cut chicken wire on a fabricated climbing structure out in the back lot.  This condition exposed the employees to struck-by and laceration hazards.

This condition exposed the employees to potential electrical hazards.
