29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Laser Craft, Inc. at the location of 38900 Taylor Parkway, North Ridgeville, OH: On or about June 16, 2021, the employer did not develop and implement an energy control program consisting of training, procedures, and audits, to protect employees from hazardous mechanical energy sources.  Employees that performed maintenance tasks such as, but not limited to, aligning tables and chains on the "T2" machines, were exposed to amputation and caught-in injuries while servicing the machines.

29 CFR  1904.29(a):The employer did not use an OSHA 300, 301 and 300A Form or equivalent.  a) Laser Craft, Inc. at 38900 Taylor Parkway N in Ridgeville, OH: On or about June 16, 2021, the employer did not ensure OSHA 300 Injury and Illness Logs were kept. A reportable injury occurred on June 16, 2021 and it was not recorded.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Laser Craft, Inc. on or about June 16, 2021, at the address of 38900 Taylor Parkway North Ridgeville, OH: The employer failed to report the amputation injuries of an employee as a result of a workplace injury/incident.  The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employee's amputation injuries within 24 hours of receiving the injuries.  The employer later contacted the OSHA Office on June 23, 2021.
