29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: (a) North Stairwell: On or about July 1, 2021 and at times prior; employees were exposed to fall hazards while on a fabricated frame scaffold installing metal studs to the ceiling of a stairwell. One section of the scaffold did not have base plates, instead casters were used and placed on stair treads. This was not being used as a mobile scaffolding. Note: Abatement certification AND supporting documentation are required for this item.

29 CFR 1926.34(a): Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied: (a) North Stairwell: On or about July 1, 2021 and at times prior; employees were exposed to burns and/or smoke inhalation if a fire occurred. Cross bracing of the scaffolding was impeding travel down the stairwell. (b) South Stairwell: On or about July 1, 2021 and at times prior; employees were exposed to burns and/or smoke inhalation if a fire occurred. The baker scaffold was placed on the stairwell leaving 17 inches of travel. Note: Abatement certification is required for this item.
