29 CFR  1910.26(c):  The employer did not ensure that portable dockboards  were secured by anchoring them in place or using equipment or devices that prevent the dockboard from moving out of a safe position. If the employer has demonstrated that securing the dockboard is not feasible, the employer did not ensure that there is sufficient contact between the dockboard and the surface to prevent the dockboard from moving out of a safe position:  (a)  Lexmark International Inc. at 6555 Monarch Road in Longmont, CO:  On and before June 27, 2021, the employer did not ensure that the dockboard used at the entrance of a storage trailer was secured from displacement.  After the dockboard shifted under the weight of a powered industrial truck, an employee grabbed the dockboard to reposition it and the movement of the dockboard crushed the workers finger resulting in a serious injury.

29 CFR  1910.28(b)(3)(i):  The employer did not ensure that each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level:  (a)  Lexmark International Inc. at 6555 Monarch Road in Longmont, CO:  On and before July 2, 2021 the employer did not protect workers from stepping into holes in the second and third floors of the pilot plant where chutes had been removed and covers had not been affixed.  Employees were exposed to trip and fall hazards.  (b) Lexmark International Inc. at 6555 Monarch Road in Longmont, CO:  On and before July 2, 2021 the employer did not protect workers from stepping into holes in the second and third floors of Jet Mill 8 where chutes had been removed and covers had not been affixed.  Employees were exposed to trip and fall hazards.

29 CFR  1910.1200(f)(6):  The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party:   (a)  On or about July 12, 2021, and at times prior, the employer did not ensure that employees were safe while using hazardous chemicals in that a bottle of tetrahydrofuran (flammable, respiratory irritant, suspected carcinogen) was unlabeled.  The contents were used regularly during the chemical processing on the drum side of the production area.  The bottle was refilled monthly and not emptied between each shift.
