29 CFR 1926.1052(c)(4): Mid-rails, screens, mesh, intermediate vertical members, or equivalent  intermediate structural members were not provided between the top rail of the stair rail system and the stairway steps: �   a. 400 Ridgedale Avenue East Hanover, NJ  On or about July 7, 2021, at the above addressed jobsite, the stairway to the scaffold stairway was not  provided with mid-rails, thereby exposing employees to fall hazards.
